Should a client opt not to provide personal information in accordance with this Privacy Policy, it may impede our ability to deliver our exercise physiology services. Clients may request anonymity or the use of a pseudonym, unless impractical or legally mandated. In such cases, consultation fees must be paid at the time of the appointment.

Purpose of Holding Personal Information

The gathering and utilization of personal information serve the purpose of delivering exercise physiology services, encompassing assessment, diagnosis, and treatment. Retention of this information facilitates session documentation and ensures the provision of relevant and informed services.

Clients’ personal information is disclosed only under specific circumstances, such as when subpoenaed by a court or when failure to disclose poses a serious risk to life, health, or safety. Disclosure may also occur with the client’s prior consent or when mandated by law.

Requests for Access and Correction to Client Information

Clients may request access to and correction of their personal information held by us. Upon receipt of such requests, reasonable steps will be taken to ensure accuracy, completeness, and currency of the information. Requests should be submitted in writing to Michael Upton, and will be addressed within 30 days.

Upon request, clients can obtain a copy of the Australian Privacy Principles detailing their rights and the appropriate handling of their personal information.
